REFERRAL PROCESS







1 / Referral
Submit a referral using the online form. This will be sent to our administration team.

2 / Initial Contact
We will get in touch with the referrer, e.g. support coordinator or client.
​
3 / Risk Assessment
We will complete a risk assessment which ensures the safety of the client and staff during home visits and community outings.

4 / Booking an Initial Appointment
We will book in an initial appointment with your assigned clinician at your preferred location and time.

5/ Booking Reminder
A reminder text will be sent 48 hours prior to the appointment (if a personal mobile number has been provided).
If you need to change or cancel the appointment, get in touch with us: info@autonomyalliedhealth.com.
We require 48 hours notice of any changes or cancellations.
6/ Service Agreement
Either before or at our first appointment, we will develop a service agreement together. This outlines what you can expect from our services.

During our first appointment, we will discuss your current goals and what you would like to achieve with us.
